The NHS in England wants to make it easier for Health professionals across the country to access important medical information about you in the event of an emergency. Knowing about any medicines you are taking, allergies you suffer from and any bad reactions to medicines you have had could literally save your life, by making sure that those caring for you have enough information to treat you safely.
At the moment, we just need to know about medicines, allergies and bad reactions to medicines. Once a record has been created for all patients in Herefordshire, there may be an opportunity to add further information, but not without your permission.
Healthcare staff will need to ask your permission before they look at your record, except in certain circumstances, for example if you are unconscious. The information gathered does not include your medical history, just details of medications, allergies and bad reactions to medicines.
NHS Herefordshire will be writing to all Herefordshire residents in a phased approach over the next three months, explaining what the Summary Care Records will be used for, and how to get more information. Although having a Summary Care Record will help NHS professionals treat you safely in the event of an emergency, you have a choice:
Yes I would like a Summary Care Record - you don't need to do anything and a Summary Care Record will be created for you from information provided by your GP.
No I do not want a Summary Care Record - complete and return the opt-out form sent to you when you receive your letter from NHS Herefordshire. Alternatively, complete the form in the resources box below and send it to your GP.
You can choose not to have a summary Care Record and change your mind at any time by informing your GP practice.
