Welcome to the Service Integration Programme
We have developed a new integrated model of care based on a number of key principles:- Deliver services through improved pathways of care;
- Deliver health and social care through local neighbourhood teams where possible;
- Focus on predicting and preventing crises in the people that are most at risk of becoming unwell;
- Provide improved services for dealing with crises when they do happen.
This site aims to tell you about the background of the Programme and describe the work that we are doing to implement the new model of care.
The work of the programme is vast and could lead to exciting new ways of delivering an integrated health and social care service in Herefordshire.
We are keen to hear your views about our work and you can email questions and suggestions to the Service Integration Programme Team through the feedback link on the right hand side of the page.
